Program Overview

  • Contact your insurance to confirm weight loss surgery is a covered benefit under your policy.
     
  • Attend Trinity Weight Loss Seminar and submit $50 registration fee money and paperwork received prior to the seminar to start the next step in our program.
     
  • The evening of the seminar you will be given health history forms to take home to fill out and bring to your appointment with Sara A. Neyens BSN,RN, CGRN, or Lynne Coffey, ARNP.
     
  • Participate in group visit with registered dietitian to go over diet choices and food labels. The Trinity bariatric team is dedicated to helping you learn more about healthy eating habits and reading labels. The Trinity team will work with you to schedule this.
     
  • After the group dietitian visit your next appointment will be set up for you to come to the Bariatric Center to go over your health history and financial responsibilities for bariatric surgery.
     
  • Attend Trinity Bariatric Center appointment with Sara A. Neyens BSN,RN, CGRN, or Lynne Coffey, ARNP for your health history and Jana Kindred for insurance review.
  • Trinity Bariatric Center requires a psychological evaluation to ensure your success after bariatric surgery. The evaluation is to determine if you will be a good candidate for bariatric surgery. Trinity will give you the information you need to setup that appointment.
     
  • Individual appointment with dietitian to go over your specific eating habits and plans for the future.
     
  • You will be provided a form for your primary care physician to fill out each month for six months to monitor your weight loss progress. Documentation will need to include specific diet and exercise plans. Trinity Bariatric Center requires you to be weight stable and not increase your weight during this six month period. Not all insurance carriers require this step.
     
  • A letter of medical necessity is a referral letter from your primary care physician stating that they support your decision to have bariatric surgery. If you currently do not have a primary care physician, you may call our office and we will be happy to refer you to one.
     
  • The Bariatric Support Group meets the third Tuesday of each month at Trinity Moline Campus. Attendance at one support group prior to surgery is required. It is important for you to attend after your surgery to meet and talk with other people who are having the same experiences as you are.
       
  • Bariatric Center will submit information to insurance company for approval. The approval process may take two to six weeks.
     
  • If surgery is approved your surgery will be scheduled. Two weeks prior to surgery you will come in for pre-surgical testing: labs and x-rays, nutritional instruction with a registered dietitian, and preoperative education with our program administrator.
     
  • Trinity encourages Bariatric patients to work with a physical therapist on an exercise program specific for the patient’s needs. This appointment is made about one month post-operative.
     
  • Trinity’s Bariatric Program requires you to meet for counseling with a psychologist one month and two months post-operative for support on your weight loss journey.
