Preparation List for Bariatric Surgery

1. Call your insurance to verify that you are eligible for this type of surgery. The number for customer service is on the back of your insurance card.
   • We will need a copy of your insurance card both front and back.
   • We will call your insurance to find out the criteria for you to meet in order to be a candidate for surgery.
2. Obtain a Letter of Medical Necessity from your family practice doctor stating support of your decision to have surgery and health related problems from morbid obesity.
3. Some insurance companies require 3 to 6 months of doctor’s office visits showing a physician supervised diet and exercise plan. 
   • Plan on scheduling monthly office visits with your primary care physician for weight management including a diet and exercise plan.
   • These visits must be consecutive, every month, up until the time of surgery.
   • Send a copy of the documented weight management visits each month to the Bariatric Center.
4. Most insurance companies require a psychological evaluation to be competed specifically for Bariatric surgery. Trinity’s Bariatric program requires you to complete a psychological evaluation.
   • You will be provided with a list of psychologists with bariatric experience. You will then make you own appointment with the psychologist of your choice.
5. Diet and Exercise Programs
   • Photo copy and submit any receipts from any commercially based programs you have enrolled in for the past 5 years i.e., Jenny Craig, Weight Watchers, etc.
